Fill out the entry form below. If you are entering more than one title, simply repeat the entry process for each title.
Step 3
Go to your inbox and print out the entry form(s). You will submit two books and two entry forms for each category entered. Thus, you will have two entry forms for each book category you entered. (Example: if you entered one book into 3 categories, you will have 6 forms to print out.)
Step 4
Insert one entry form in the front cover of each book. Remember, you will submit two books and two entry forms for each category entered.
Step 5
Mail your books to the address on the entry form. Multiple titles may be included in the same package. Just be sure each book has an entry form inserted inside the cover flap.
